Applying to Mistwood

To place your child on our waiting list, please print and fill out the following two items (use buttons below to link): an Enrollment Application and a Parent Questionnaire. Yes, the forms are not fillable, we’d love for you to print the forms and fill them out in your own handwriting. Your intentional attention here will help you to connect with our program from this very first step. Once you have filled out your application, please call or email us to schedule a tour of our school. You can bring your application when you come for the tour, or if you’d rather get it in right away, you can mail it to us. There is a non-refundable $100 Application fee due with this paperwork, payable by check / cash OR we can send a digital invoice. We ask that your first tour be without your child, so that you are free to take in the information and environment while you’re here at Mistwood. Once we have begun the enrollment process we will of course set up a time for your child to visit the classroom!
